Root cause analysis is intended to ensure that the cause of a problem is resolved rather than just a symptom. One technique for visually identifying a root cause is the Ishikawa diagram, also known as a Fishbone Diagram or Cause & Effect Diagram. Another technique is the "5 Whys".
