The PMBOK glossary defines the scope management plan as a "component of the project management plan that describes how the scope will be defined, developed, monitored, controlled, and verified."

The scope management plan is created by (output of) the plan scope management process. It is used by (input to) the develop project management plan process as well as collect requirements, define scope, and create WBS processes.
